J-1 EV Scholar Sites of Activity
J-1 Exchange Visitor Scholars should only engage in activity at sites listed on their Form DS-2019.
Primary site of activity Heading link
Your primary site of activity is listed in Box 2 of your Form DS-2019. At UIC, this will be the main address of your host campus in either Chicago, Rockford, or Peoria.
Updating your sites of activity Heading link
In some cases, you may need to conduct program activity at a location outside your primary site of activity. For example, you need to leave campus to temporarily pursue program objectives at an affiliated or third-party hospital, health clinic, research facilities, training site, etc. In these cases, you must communicate the anticipated update to your site activity to OIS in advance and the activity must be integral to your primary EV program objectives. All such activity may not delay your original program activity, and be approved by your supervisor. This includes one-time engagements or brief periods of stay.
To report a change or additional site of activity, please complete the Add a Site of Activity E-Form in myOIS. An OIS advisor will contact you within 3 business days to let notify you if any further information is required.
