J-1 EV Scholar Program Extensions

If you find that your UIC DS-2019 will expire before you complete the objectives of your J-1 Exchange Visitor Program, you will need to work with your host department to request an extension with OIS.

To extend your J-1 Exchange Visitor Program, your UIC department must initiate the extension request process with our office by submitting an extension request form and supplemental documents to OIS.

Extension requests must be submitted at least 60 days prior to your current program end date and the program objective/subject field code from Box 4 on your Form DS-2019 must remain the same. Please ensure you have active and valid health insurance on file with OIS when applying for your program extension.

If you have applied for a waiver of the 212(e) Two-Year Home Residency Requirement, please contact OIS prior to submission of your extension request, as you may not be eligible to extend your current program.

OIS will also issue a travel authorization signature for you and any active J-2 dependents when we issue your extended Form DS-2019.
